About the Role:

An empathetic and creative thinker, theCOVID-19 Case Managersupports community members requiring resource assistance and other support. The COVID-19 Case Management program is an arm of the Pandemic Health Navigator Program (PHNP), a grant-funded, short-term program focused on reducing the disparities of outcomes of COVID-19 seen in our community through education/outreach, resource coordination, and contact tracing support. COVID-19 Case managers receive referrals, complete intake interviews to evaluate needs, provide ongoing resource coordination/support for various health and social needs (e.g., primary healthcare, transportation, housing/rental assistance, food support, etc.), and register and schedule appointments in the AllVax system. COVID-19 Case Managers work closely with Contact Tracers, Community Health Workers, Health and Social Service providers, and Community Based Organizations to provide wrap-around services that encourage safe and healthy behaviors.

This position is grant-funded through December 31, 2021.

Location: 3010 Grand Ave, Waukegan, IL

Hours: 40 hours a week; Tuesday-Saturday or Sunday -Thursday and some evenings

FLSA STATUS: Hourly

Skills and Experience

  • Build rapport and connection with clients in an empathetic manner.

  • Receive referrals from Case Manager Lead and initiate contact with 90% of contacts within 24 hours and 100% of contacts within 48 hours.

  • Attempt a minimum of 3 phone attempts in the first 24 hours, and a minimum of 6 phone attempts in the first 48 hours. If unable to contact the case by phone, refer to a Community Health Worker for in-person support.

  • Provide intensive case management to connect clients with needed resources.

  • Screen clients for Social Determinant of Health needs such as meals, medicine, mobility support, immigration matters, work and income resources, rental/utility support, mental health support, support for unsafe living conditions (e.g. domestic abuse), and make service referrals or enroll clients in services as appropriate.

  • Assist clients in scheduling, accessing, and utilizing telehealth services with their primary care provider. If the client does not have a PCP, work to get the client established with a PCP.

  • Assist clients in registering and scheduling a vaccination appointment in the AllVax system, including arranging transportation support where needed.

  • Provide clients with resource information by utilizing local database systems and the IPHA Resource List.

  • Document 100% of efforts in Salesforce or AllVax the same day these activities occur.

  • Participate in all required meetings and trainings related to the PHNP.

  • Cross-refer clients to relevant agencies for additional support.

  • Job Skill Assessment(s) may be required for this role.

Education, Qualifications, and Certification:

  • High School Diploma is required.

  • Will provide on the job training.

  • Preferred experience in public health, community health, or case management.

  • Excellent oral and written communication skills.

  • Comfort with using technology to communicate and track activities.

  • Must have experience working in community work. This can include advocacy, volunteering, etc.

  • Ability to communicate effectively with educational and culturally diverse populations in a courteous and professional manner.

  • Ability to plan and organize work effectively and efficiently.

  • Case management experience is a plus.

  • Valid driver’s license.

  • Bilingual is a plus.
