Mercy Medical Center Clinton Social worker - MSW, Case Management, Full-time in United States
Provides psychosocial and supportive intervention, consultation and education to patients/families to assure comprehensive services throughout a patient's hospitalization, leading to a successful transition for discharge or transfer. Utilizes the Social Work process to determine individual patient needs and appropriate community resources to assure continuity of care from hospital to home or another healthcare facility. Contributes to the training of Social Workers and other health care professionals with resources, a discharge plan and assistance as needed during evenings, weekends, holidays and/or in the absence of an assigned day shift Social Worker.
Responds to immediate requests and ensures a comprehensive medical plan by communicating the psychosocial and/or discharge information to the medical team verbally and through documentation in the patient's medical record.
Assesses the patient's mental and social functioning by providing a psychosocial screen/history, assessment, resource information, counseling and a discharge plan.
Completes discharge plan requirements, i.e. appropriate transport forms, authorization for release of medical information, contacting appropriate family or agency representatives, etc.
Adheres to the Social Work Code of Ethics and ensures on-going professional education to provide state of the art psychosocial and discharge care.
Meets the Department of Social Work management accountabilities of attendance at unit Staff meetings, completes medical record chart notes and statistical reports, completes LUMC mandatory education classes and regulatory compliance, etc. Attends service line multidisciplinary
rounds and meetings specific to the assignment responsibility tofacilitate continuity of patient care.
Required: Masters Degree

Preferred: 1-2 years of previous job-related experience
