Beaumont Home Health and Hospice is looking for a RN Team Manager, Hospice to join our team!

The RN Team Manager, Hospice assumes accountability and leadership for hospice functions, as well as quality to achieve clinical excellence outcomes, regulatory compliance, patient and employee satisfaction in a manner that supports fiscal responsibility and profitable growth. The Hospice RN Team Manager provides oversight for the program office operations, staffing, and patient care in their team geographical areas. They collaborate with the Clinical Director, the Executive Director, and Regional VP of Care Integration, to ensure continuity of services and company growth and profitability.

  • Provides oversight for the overall hospice process for quality & safe patient care of the patient from beginning to end of episode

  • Collaborates with corporate and regional leadership the implementation of policy, procedures and quality improvement programs related to hospice

  • Organize and allocate clinical staff to carry out the activities necessary to accomplish clinical and service objectives

  • Provides appropriate level of support and coaching related to hospice care

  • Monitors trends and quality indicators of hospice care to ensure workforce needs are met with collaboration of corporate and local leadership

  • Meet and exceed quality indicators with staff qualifications and hospice services

  • Assists in the projection of the number, type and use of personnel for current and future needs for the agency in regard to hospice services in collaboration with corporate and local leadership

  • Monitors patient and geographic center metrics and trends in utilization and outcomes of hospice to achieve specific results for company focus areas aligned with Mission and Vision

  • Assists in the allocation of clinical staff and clinical support staff to carry out the activities necessary to accomplish the hospice objectives

  • Sponsors hospice procedure and quality improvement programs in the field and field support programs

  • Collaborates with local executive director of Hospice and corporate leadership to ensure continuity of services and capacity for growth

  • Maintain professional growth and development through seminars, workshops and professional affiliations to disseminate Best Practices throughout organization and maintain competitive edge

  • Assumes accountability for execution of all documentation and clinical practices in accordance with agency protocol and Medicare/Federal guidelines

  • Maintains expert level of industry knowledge of hospice services

  • Ensures all hospice policies and procedures for compliance with local, state and federal regulations

  • Maintains active RN license in state of employment

  • Consistently demonstrates Core Values and maintains a professional appearance and Cultural Fit as a representative of the company

  • Provides direction and supports with regards to front line supervisors

  • Performs other projects as assigned

  • This position requires a Registered Nurse, training and experience in hospice care

  • Three years of supervisory or administrative experience in hospice or related health programs preferred

  • Expertise and Hospice background for multiple sites preferred
