$7,500.00 Sign-On Bonus
JOB SUMMARY: To admit patients to skilled home care and home hospice programs. Develop plans of care in collaboration with patients, families and physicians. Provide new patients with information on home care and hospice services and provide intial patient education.
-Accurately and thoroughly explains home care and home hospice services to prosepctive patients/families in an understanding manner. Explains insurance benefits and coverage of services, patient responsibilities and options for alternative means of payment.
-Coordinates the admissions process and completes paperwork accuratelly within 24 hours of admission.
-Develops the intial plan of care based on input from patient, family and physician. Obtains physician orders for other appropriate disciplines as needed.
-Notifies scheduling and other disciplines of new evaluation orders via the task system within the electronic health record.
-Utilizes OASIS scrubber software to verify OASIS accuracy and consistency.
-Develops and maintains OASIS and HIS proficiency.
-Completes pre-admission evaluations and admissions into the skilled, hospice and bridge program as well as some routine and death visits.
-Facilitates transition of patient/family to the case manager and other members of the interdisciplinary team.
-Coordinates the patient's discharge from the hospital and admission to hospice including transportation, equipment, medications and supplies as needed.
-Makes home/SNF visits as needed to admit patients to hospice and assist with routine visits or carries a small caseload of patients as needed.
-Maintains proficiency in use of the electronic health record.
-Completes quarterly inservices within the timeframe permitted.
