As a Coord Health Information at Dycora, you will ensure that our patients get the care they need and that we have the necessary resources to provide it.   • Medical Record Maintenance to include (but not limited to) assembly, maintenance and retention of Active, Thinned and Closed Medical Records

  • Medical Record Audits to include Admission/Readmission/21 day audits, Quarterly Audits, Discharge Audits, Physician Visit Monitoring and assigned Focus Audits

  • Quality Reporting to include weekly reporting of audit findings as well as monthly Quality Assurance and Assessment reports

  • Medical Record Tracking to include filing, storage and retention of Active, Thinned and Closed Medical Records and Health Information destruction procedures

  • Maintenance of the Master Resident Index to include both In-house and Discharge Resident Information

  • Administrative Functions to include (but not limited to) the Release of Resident Information and coordination with other staff related to the development and maintenance of the Signature Legend and Master Forms Manual


  • High School Diploma or equivalent

  • College degree, preferred

  • Certification in Health Information Management, preferred

  • Experience in long term care, preferred

  • Must be capable of maintaining regular attendance

  • Self-motivated, organized, and detail oriented

  • Basic computer knowledge and ability with an aptitude to learn company software

  • Ability to manage multiple conflicting priorities

  • Ability to perform critical thinking, analysis, and problem solving

  • Must have good customer service and telephone skills

  • Must be able to maintain confidentiality regarding patient and company proprietary information

  • Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
