About the Position
Triaging patients in a customer service center, mostly consisting of telephone triage, walk-ins also occur, includes patient education, and collaboration with Primary Care Teams.
Schedule: Monday - Friday; 8:00 am - 5:00 pm

About the Division
The Community Health Centers have been providing quality primary care and behavioral health services to the citizens of Lane County since 2004. We provide a broad array of services to adults and children in an outpatient setting. The CHC is the primary source for uninsured and underinsured. Our clinics are Federally Qualified Health Centers whose mission is to serve the community by offering excellent care and eliminating barriers to healthcare.

Examples of Duties - Duties may include, but are not limited to the following
Utilizing independent judgment, assesses plans, and evaluates individual, family and community health needs.
Plans and implements general public or community health nursing services in an assigned geographic area of responsibility or special program.
Plans and directs clinics or special programs with clinics.
Arranges or conducts health classes or group meetings with teachers, school nurses, parents, industrial groups, and expectant parents.
Performs home or office visits, explaining basic concepts of prevention of disease and promotion of mental and physical health.
Assists families in understanding health needs and methods of meeting needs through education, direct service and referral to appropriate health resources.
Plans and exchanges information with physicians and other health workers for more effective care of the individual patient.
Organizes and coordinates the development of community organization and planning for health services with community groups.
Schedules visits with school administrators and teachers in a district and assists them in identifying health problems and interpreting medical reports and recommendations.
Confers with pupils, teachers, administrators, school nurses, parents, and physicians on specific health problems.
Participates in conferences with other disciplines and agencies to coordinate services and plan joint action.
Maintains records for nursing services to families, schools and clinics and makes reports as required.
Organizes trains and supervises volunteer workers.
Provides public and community health nursing services in special settings such as hospital coordination, disease investigations and mental health and drug programs.
Dispenses medication under standing orders of a physician, and supervises and monitors its effects on a patient.
Collects laboratory samples and interprets their results where applicable.
Practices, principles and theory of public and community health nursing.
Laws, rules and regulations governing the provision of public and community health care nursing.
Community resources available for public and community health nursing problems.
Community and state health standards and practices.
Illnesses and diseases associated with public health, and the treatments available for such diseases.
Practices and operating characteristics of available public and community health programs.
Policies and protocols of program and clinic operations.
Practices, principles and policies of public and community health education.
Examination and assessment techniques used to analyze a variety of health situations including sexually transmitted diseases, pregnancy, tuberculosis, communicable diseases, immunization status and risks, maternal/child health and other public health situations.
Independently assess, identify and treat illnesses and diseases associated with public and community health nursing services following medical standing orders and acting within nursing scope and standards of practice.
Interpret laws, rules and regulations as applied to health care and education.
Identify, use and coordinate with community resources and other public health care programs.
Identify problems and generate solutions within the scope of the department, division, program or clinic.
Provide educational services to target populations, patients, and the general public.
Determine needs of the community as applied to public and community health nursing services.
Examine patients and assess their physical well-being.
Establish and maintain effective working relationships with those contacted in the course of work.
Communicate clearly and concisely, both orally and in writing.
Establish, maintain and update a variety of reports, charts and documentation associated with public health care.
Experience and Training; Additional Information
Bachelor of Science in Nursing degree from an accredited college or university.
Nursing experience in public health or community health care services desirable.
An equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.
• Possession of a current State of Oregon Registered Nurse license at time of appointment.
• CPR/BLS by time of hire
• Immunization records and TB test within 10 days of hire
