JOB SUMMARY: Responsible for the oversight of daily operations, staffing and workflows for the Care Management Department at Northwest Hospital, including 20 direct reports comprised of social workers, RN inpatient care coordinators and PRN staff. This position will report to the Director of Care Management. In collaboration with the hospital-based leader, this professional manages a team of professional staff, composed primarily of nurses and social workers. This team is responsible for providing care management services to the hospital's patient population that consists of care coordination, care progression, discharge planning, medical necessity reviews, and psycho-social services. This team also works in collaboration with the medical and nursing staff to manage length of stay and the care of patients at risk for readmission. Under the Director's guidance, the Manager leads the team in continuous process improvement activities to ensure that patients receive cost-effective, high quality, and safe care. This professional also collaborates with both hospital and care management leadership to create and achieve department metrics that reflect the hospital's financial performance and progress toward patient satisfaction goals. As a member of the leadership team, the incumbent serves as a role model that supports professionalism, service, positive communication, teamwork and safety.
REQUIREMENTS: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Bachelor's in Nursing. 3-5 years of experience. Maryland Registered Nurse License, CCM or ACM preferred. Certification within 3 years. Prior experience with Lean Process Improvement methodology is a plus.
