Reference #: 9233884PRIMARY FUNCTION:The position will provide support to the Director of Primary Care Integrated Behavioral Health in advancing and sustaining implementation efforts for integrated behavioral health services across Jefferson Health practices, with a particular emphasis on coaching practice teams to incorporate and build on successful primary care integrated behavioral health strategies. Under the direction of the Project Director of the Jeff Be Well program and the Director of Primary Care Integrated Behavioral Health, IBH Practice Transformation Coach will assist the onsite Behavioral Health Team with clinical functions of the department, consistent with corporate, administrative and clinical policies and procedures. The IBH Practice Transformation Coach works with primary care practices with assigned behavioral health consultants to improve quality of care. This position coaches practice staff to improve patient outcomes by developing skills in process improvement, team-based care and encouraging patient engagement. Providing leadership and expertise in the development, implementation, and optimization of training materials used to facilitate practice transformation. The IBH Practice Transformation Coach will work as part of an interdisciplinary team to create and deliver products and services including user education, training materials and tool kits. The IBH Practice Transformation Coach is also a practicing clinician who provides limited direct patient care to an assigned practice. The IBH Transformation Coach works to maintain high quality clinic service delivery, efficient work performance, and excellent customer service within the department and across practices.ESSENTIAL FUNCTIONS:Provide customized coaching at clinical sites by working with practices to identify practice- specific needs and opportunities, prioritizing areas of focus, and identifying available and needed resources.Prioritize system improvement efforts. Identify and facilitate opportunities to integrate and align projects with other related practice transformation initiatives.Collaborate with practices to facilitate redesign efforts, support IBH care teams, identify resources to address day-to-day workflow issues and help with peer coaching.Serves as an Integrated Behavioral Health Department Champion to ensure the integration and implementation of evidence-based behavioral health practices within the respective practices.Advise individual clinical sites on the implementation of the IBH model, sharing specific best practice examples.Address barriers to effective implementation of improvement processes; propose solutions for practices to consider in addressing barriers.Serve as a liaison to practices to provide on-site and remote assistance and facilitation to the clinicians, care teams and their associated practices to drive improvement in IBH implementation, education and operational efficiency.Work closely with the Director of Primary Care Integrated Behavioral Health and designated management staff to address IBH workflow issues that may arise and develop a plan for improvement.Prepare practices to continue the transformative activities that demonstrate consistent improvement on performance and behavioral health integration.Provide effective ongoing monitoring.Assists in the preparation of reports and written documentation related to behavioral health service delivery, clinic activities, and/or patient care outcomes.Maintains clinical patient care duties as a Behavioral Health Consultant.Maintains productivity expectations in accordance with clinic/program guidelines.Participate as a member of the health center staff in the event of a clinical emergency.Meet regularly with Director of Primary Care Integrated Behavioral Health to discuss standards of care, service delivery innovation and integration, patient/provider satisfaction, team performance and related behavioral health and patient care outcomes.Execute responsibilities in a
