The Resource Center Coordinator is accountable for facilitating, implementing, and confirming post acute transition plans for the Fisher-Titus Medical Center patient population. The position will respond to Hospital and community-based service requests from the nursing staff, the case managers, the social service workers, therapists, hospitalists, or private attending physicians practices.
High School Diploma or equivalent.
Associate Degree preferred.
Basic Knowledge of medical terminology or clinical experience.
Hospital experience is essential and evidence of effective interpersonal communication skills is required.
Must be able to demonstrate the ability to: Work independently, Prioritize tasks, Communicate effectively demonstrating both verbal and written skills, Gain proficiency in the use of computer systems and software.
