Position Summary:
We have an exciting opportunity to join our team as a Sr Placement + Rehab Counselor.

In this role, the successful candidate Position Summary:Provides vocational rehabilitation and related support services including counseling, assessment, training and job placement to individuals with physical, neurological, and psychiatric disabilities under minimal supervision.

Job Responsibilities:
Assesses client statusand formulatesappropriatevocational goal andplan. Interpretsmedical, social andpsychologicalinformation andevaluates patientsvocational history,current status, andvocational options.Determines ifadditional informationis required forplanning and referspatient to obtainrequireddocumentation.Demonstrates goodjudgment inselecting,interpreting, andreporting data.Utilizes effectiveassessmenttechniques.
Provides individual/group rehabilitation counseling services based on the needs of the patient. Establishes a rapport with the patient and explains the role of the rehabilitation counselor and vocational services. Develops a therapeutic and supportive relationship with the patient and maintains an ongoing productive counseling relationship. Demonstrates sensitivity to changes in patient status and modifies treatment plan accordingly. Takes initiative in identifying problems and developing treatment strategies.
Implements andcoordinates clientstraining program.Orients work sitesupervisors to thegoals of the trainingprogram and theirresponsibilities whenworking with programparticipants.Develops andmaintains contactswith Medical Centerdepartments andexternal organizations forplacement of trainingand internshipprogram participantsat appropriate sites.Develops newtraining sites asneeded to meetpatient needs.Provides on-siteconsultation,supervision, and jobcoaching as requiredand uses professionaljudgment andcreativity to developinnovative solutionsand treatmentstrategies to resolveproblems. Maintainstimely contact withother treatingclinicians and/orreferring counselorregarding clientsprogress.
Acts as a liaison withreferringorganizationsincluding ACCES andthe VA, physicians,insurance companies,and advocacyagencies. Initiatesreferrals, whenindicated, in a timelymanner. Coordinatesactivities withcommunity basedprograms. Representsthe department atACCES and otherorganizationsmeetings, seminars,conferences, vendorfairs, and speakingengagements.Demonstratesknowledge,resourcefulness, andconfidence indeveloping liaisonrelationships withindividuals in otherprograms to developa network ofprofessional contactswhich facilitates theprovision ofvocational servcies topatients.
Provides effective job placement and follow-up services. Presents clients to the Business Advisory Committee. Provides individual/group job seeking skills training. Prepares effective resumes and cover letters, teaches interview strategies. Provides client with appropriate job postings and referrals and supports clients job search. Provides post-placement follow-up with client and employer.
Develops and maintains an employer network. Initiates employer contacts and markets program and candidates. Develops and follows-up on job leads. Attends Placement Consortium meetings. Performs job market research and surveys new employers in growth industries. Recruits members for the Business Advisory Committee.
Documents allservices andcompletes allprogram reportsusing clear, concisenarrative followingprescribed protocoland withinestablishedtimeframes. Insurescase notes reflectpatient status andprogress towardgoals. Entersprogress notes andreports into Epic.
Participates actively in department projects and performance improvement studies.
Supervises student interns when available.
Completes monthlystatistics according todepartmental timeand proceduralguidelines. Submitstimely external travelnotification onappropriate form andreports safetyoccurrences on PSN.
Demonstrates continued proficiency in skills required to treat assigned patient population.
Performs other duties as assigned.

Minimum Qualifications:
To qualify you must have a Minimum Qualifications:Masters degree in Rehabilitation Counseling or related field and two years related experience.Rehabilitation Counselor Certification.Word processing, spreadsheet, and presentation software skills; knowledge of Internet resources. Preferred Qualifications:
Preferred Qualifications:Masters degree in Rehabilitation Counseling, Rehabilitation Counselor Certification and three years experience, at least one of which involved providing direct job placement services to individuals with disabilities. Knowledge of resources for job opportunities including Internet sources and an established network of employer contacts.
Qualified candidates must be able to effectively communicate with all levels of the organization.
