JOB REQUIREMENTS: Under the direction of department leadership, the Referral Coordinator is responsible for managing the daily referral activity, both through new and ongoing referrals. Collaborates closely with the Organ Donation Specialists and donor hospital staff to obtain information, triage the referral and determine a plan for ongoing management with AOC. DUTIES AND RESPONSIBILITIES *Demonstrates Core Values: Do Right Always, Find a Better Way, Make Results Happen and Strengthen the Team. *Responds to initial referral from outside call center *Calls hospital staff back within a timely manner, obtains initial patient information and documents all interactions in the electronic record *Performs daily check in calls and electronic record reviews of existing referrals *Collaborates with AOC to triage referrals and create a plan for most appropriate next steps *Performs chart reviews and data entry into the electronic record *Responds to referrals onsite as directed by AOC *Huddles with hospital staff to obtain additional information and documents all interactions in the electronic record *Supports the hospital staff through the referral process and provides additional education and information surrounding the donation process *Engages with full awareness and sensitivity to cultural and religious beliefs regarding the grief process *Assists Hospital Development Coordinators with educating health care professionals and the public regarding organ donation; may develop, conduct, and participate in education programs for health care professionals and the public *Contributes to quality assurance activities *Supports and demonstrates a customer service commitment to internal and external customers through adherence to Versiti customer service standards and processes *Maintains competence through attendance job-related continuing education *Actively participates and contributes in Lean and continuous improvement *Understands and performs to all applicable regulatory and compliance requirements set forth by AOPO (Association of Organ Procurement Organizations) and UNOS *Demonstrates schedule flexibility as determined by business need *Work/job related travel and reliable transportation is required *Requires evening, weekend and holiday work hours *Proactively seeks continued professional development Education *Associate degree or certification in related healthcare field Experience *Previous experience working in healthcare, familiarity with medical terminology *Minimum 1 year of related work experience preferred *Experience working with grief, loss, or end of life care...
