Humana Care Manager, Telephonic Nurse 2 (Work at Home - ID, UT, OR, WA) in Meridian, Idaho


The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

  • Focusing on social determinants of health

  • Closing gaps in care by identifying and resolving barriers that hinder effective care

  • Act as an advocate on the member's behalf by providing resources and patient education to help maintain a healthy lifestyle by continuously monitoring patient care through assessments and/or evaluations

  • Foster an environment that can provide the necessary and quality healthcare for our members to allow them to live out their best life

Required Qualifications

  • Licensed Registered Nurse (RN) in the (appropriate state) with no disciplinary action

  • 3-5 years of clinical acute care experience

  • Experience with case management, discharge planning and patient education for adult acute care

  • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook

  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences

  • Ability to work independently under general instructions and with a team

  • Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required

Preferred Qualifications

  • Education: BSN or Bachelor's degree in a related field

  • Managed care experience

  • Experience working with Medicare

  • Certified Case Manager (CCM)

Additional Information

  • Hours for this role are: Monday-Friday 8am-6pm MST (Your 8 hour shift will fall between this time)
