JUNEAU CO GOVT CCS Service Facilitator in MAUSTON, Wisconsin
JOB REQUIREMENTS: The purpose of this position is to provide Comprehensive Community Services (CCS) to the citizens of Juneau County, including providing case management and individualized services to children and adults with diagnosed mental illnesses or substance use disorder. Responsible for program compliance with all State and Federal regulations and ensures meeting requirements for continued State certification for CCS funding. This is a demanding, stressful position which requires one's full attention. Therefore, all employees accepting this position must disclose and obtain permission for any other employment activities, especially those of similar nature and function. -Considerable knowledge of human development; mental illness and substance use disorders; family, group, and community dynamics; and systems theory. -Knowledge of the Comprehensive Community Services (CCS) program. -Knowledge of the Wraparound model for services. -Strong ability to implement interpersonal and team building skills. -Strong ability to communicate effectively in writing and orally. -Knowledge of community resources and various prevention techniques. -Ability to establish rapport with clients. -Ability to exercise good judgment. -Conduct casework interviews to obtain pertinent social history data. -Implement consumer/client driven service plans based on clinical, medical necessity and in conjunction with Mental Health/Substance Use Professional -Provide case management and individual recovery based services to assigned consumers. -Coordinate services for clients, including admission, transition and discharge. -Facilitate strength-based recovery teams for clients. -Provide individual skill development with individuals, couples, groups, and families, within the office, home and community setting. -Provide consultation and collaboration with community agencies and other professionals. -Provide educational and prevention services with community groups. -Complete required record keeping according to state and agency requirements. -Participate in in-service training -Participate with other staff members in the diagnostic process and treatment planning. -Involvement with needs assessments, program development, prioritization and evaluation. -Ability to intervene and prioritize crisis situations and complete mandated reporting. -Create crisis and safety plans including de-escalation, intervention and prevention steps to avoid unnecessary out of home placement and/or hospitalization. -Respond to emergency situations in the office and community, including crisis on-call. -Other duties as assigned. ***** OTHER EXPERIENCE AND QUALIFICATIONS: -Bachelor's degree in social work, psychology or human services from an accredited undergraduate school, or a Master's degree in social work, counseling, psychology, marriage and family therapy from an accredited graduate school, preferably with eligibility for licensure. -Prior CCS experience preferred, but not required. -Must possess valid driver's license and vehicle insurance, and have access to a reliable vehicle. ***** APPLICATION INSTRUCTIONS: Other: Application and Resume to be received by 4:00 p.m. March 8, 2021 Applications can be obtained on the Juneau County Website; www.co.juneau.wi.gov Mail to: Juneau County Personnel 220 E.
