In an environment of continuous quality improvement, the Registered Nurse is responsible for providing rehabilitation and disease management through patient education and cardiac monitored exercise conditioning. The cardiac rehab nurse works with the Medical Director to develop an individual treatment plan (ITP) for each patient in the program. Interacts with physician for medication management, cardiac related events /issues and reduction in readmissions. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.
? Current licensure in the State of Ohio as a Registered Nurse
? Minimum of 1 year critical care experience required
? BLS and ACLS required
? Verbal, clerical, numerical, and manual dexterity, color vision and near visual acuity
Assesses patient need for cardiac rehabilitation and disease management by interviewing patients, performing limited physical examinations, reviewing and evaluating existing clinical data for admission criteria and assisting the Medical Director in the development of an individual treatment plan (ITP) by recommending the appropriate type and frequency of exercise, education, diagnostic testing and treatment, including goals.
Initiates, conducts, and evaluates efficacy of therapeutic procedures/interventions, medications and exercise and recommends changes to the ITP for goal achievement and successful outcomes.
Performs cardiac monitoring, diagnostic procedures, reviews test results, determines reliability, performs quality control, evaluates implication of test results and monitoring and makes clinical and critical decisions accordingly.
Prepares patient for self- management: teaching diet, exercise and medication management. Makes referrals accordingly.
Responsible for the operation verification procedures, cleaning/disinfection, calibration, set-up, maintenance, and troubleshooting of equipment used for cardiac rehab.
Maintains par inventory levels for supplies.
Assists physicians and/or performs special programs.
Educates patients and family on medications, quitting smoking, diet, exercise and disease management (Stop Light criteria).
Uses clinical and critical thinking skills to manage, treat and respond to needs of the patient.
Independently coordinates the cardiac rehab program including the maintenance program.
Acts as a liaison between patients and physicians for improved communication and readmission prevention.
Responsible for accurate documentation, billing procedures, patient tracking, reports, audits.
Actively participates in quality and safety through tracking, reporting and recommendations.
Responsible for scheduling and insurance verification.
Maintains compliance in RQI.
Assumes all other duties and responsibilities as necessary.
