The Transplant Patient Navigator will support transplant nurse coordinators and physicians with patient care management as it relates to pre-transplant recipient and donors and the transplant program. Coordinates patient care under direction of the clinical team including but not limited to:
Coordinating patient testing and scheduling
Reviewing clinical correspondence (i.e. lab results) for appropriate action
Transplant database entry
Monitoring patient lists for compliance with monthly blood specimens
Obtaining patient medical records and assisting the RN or other team members with patient care issues such as blood draw or vital signs
Develops relationships with referring physicians, consultants, affiliates and office staff
Associates degree or Bachelor s degree (preferred).
One to three years of related experience within the healthcare field
Ability and interest to learn clinical information and criteria
Knowledge of medical terminology
Emphasis on computer literacy and communication skills, such as Microsoft Excel and Word
Successful completion of all training and orientation programs
