St. Luke's Health System Case Manager (LMSW) in Ketchum, Idaho
St. Luke’s Health System in Ketchum is seeking a Case Manager (LMSW) to join our Cse Management team.
Works as consultant, educator, or community liaison to facilitate collaborative coordination of patient care.
Identify and document patient and family needs related to treatment goals.
Evaluates patient's progress towards identified goals.
Assists the care team by identifying potential barriers that impedes access to care. Implementing plan to address barriers.
Develops patient care plans, evaluates and monitors progress, responds to patient and family needs and acts as an advocate.
Coordinates discharge and post-discharge requirements for transitions of care and provides educational materials and resource management.
May provide coordination of primary care and sub-specialty providers.
Serves as subject matter expert within specialty area.
Perform other duties and responsibilities as assigned.
Education: Master’s Degree in Social Work.
Licenses/Certifications: Licensed Master Social Worker (LMSW) in state of practice.
