The primary role of the Case Manager Director is to supervise and provide support, guidance, and training to Case Managers and the Transporter & Liaison positions and manage a modified caseload in accordance with the Organization’s and licensing standards, and federal, state, and local regulatory requirements. Work Standards
Provide all Case Managers and Transport & Liaison staff with ongoing support, coaching, and training to ensure they are meeting performance goals and the needs of the youth, parents, and families
Maintain a partial caseload and provide case management services that meet the needs of the youth and is in accordance with Agency policies, licensing and funding standards, and other federal, state, and local regulatory requirements
Ensure the privacy of all youth information and case files; always comply with HIPPA/HIPAA guidelines
Ensure case managers are serving as a liaison between youth and their families; update legal guardians on youth care and progress on a weekly basis or as needed; assist youth in initiating and maintaining healthy family relationships
Ensure safety, security and sanitation standards are always maintained in all work areas
Prepare and attend supervision, unit, team, and facility meetings as required
Utilize clinical knowledge and skills, conduct assessments to determine youth needs, including but not limited to behavioral interventions, basic needs, and possible barriers to achieving individual service plan goals; identify specialized counseling and case management service needs, mental and emotional health concerns, and family reunification issues

Communication
Review case plans completed by Case Managers for accuracy, timeliness, and adherence to licensing and regulatory requirements
Review case files and notes to ensure they are accurate and completed in a timely manner and meet the Organization’s standards, state licensing standards, and other federal, state, and local regulatory requirements
Establish and ensure staff follows a formal process for case reviews and referral sources that appropriately meet the needs of the youth, and family
Review, update and maintain accurate records of all youth interactions, contacts, behavior issues, and youth progress; collect and compile relevant case history information; ensure all case files include the required documentation and required forms; maintain case files in accordance with Agency policies, licensing standards and other federal, state, and local regulatory requirements; ensure all case files are always audit ready
Respond to escalated situations and serious incidents in a timely manner and properly document the incident and outcome

Teamwork
Assist with youth placement and post-placement services
Assist case managers serving as an advocate or liaison between the youth and the court system; including accompanying youth to court appearances, as needed
Assist in the dorm by providing direct supervision, interaction, and observation of all youth while in their living space or common areas; always know the whereabouts of all youth during the shift
Interact, observe, and supervise youth in accordance with their individualized treatment and behavior support plans
Assist Case Managers and work closely with the clinical and program teams to assess social, educational, and emotional needs of the youth and devise a service plan or behavior plans to meet those needs; develop, write, and update service plans, reports, and correspondence
Aid with crisis intervention and intense case management services based on youth need; report serious or significant incidents to the appropriate parties in a timely manner; complete the necessary paperwork and provide the required documentation of the crisis or situation
Assist Case Managers and Transporter & Liaison staff with the active participate in the collaboration with interdisciplinary teams to share information, best practices, resources, and problem solve youth challenges and barriers
May be required to complete driving duties as driver and/or accompanying during transport. Subject to being designated as an authorized driver
Provide emergency and on-call services when needed

Work in collaboration with the interdisciplinary team to develop and implement specific activities in the areas of social skill acquisition, health maintenance and appropriate nutritional needs
Work closely with case management staff, interdisciplinary teams, and other program service providers to ensure residents are receiving the full scope of available program services and resources to meet Child’s Plan of Service (CPOS)
Respond to youth needs and concerns with compassion and empathy in a timely manner
Work closely with other members of management, staff, and service providers and professionals to ensure foster care children, parents, and families are receiving quality and timely services

Work as a team player and exhibit professional behavior and a positive attitude with both internal and external clients that reflects positively on the Agency and is consistent with Agency policies and practices

Provide ongoing support and coaching to all direct report staff to ensure the department is operating effectively and efficiently in accordance to established goals and objectives and program services are being delivered in manner that meets the needs of the foster care children, parents, and their families
Interview, hire, and train new staff and develop and implement ongoing staff development trainings
Coach and support staff to ensure they have the necessary tools and resources to optimize performance and ensure established project goals and deliverables are achieved on time and within budget
Regularly review employee performance and provide supportive feedback
Conduct 90-day and annual evaluations per established completion date
Minimum Qualifications
Bachelor’s degree in Social Work, Counseling, Psychology, or closely related field
3 years’ previous case management experience
Prior supervisory or management experience
Knowledge of psychological theory, methods, and ethics
Strong working knowledge of motivational interviewing
Strong writing skills with the ability to document youth care and progress properly and accurately
Ability to follow instructions and work independently and within a group and with minimum supervision
Strong organizational and time management skills with the ability to meet deadlines
The ability to relate positively to young people with serious behavioral and emotional disturbances
Strong and effective communication (verbal) skills with the ability to clearly communicate thoughts and provide instruction
Experience managing aggressive and verbally abusive behavior
Strong interpersonal skills with the ability to apply compassion and empathy, while dealing with stressful and difficult situations
Strong interpersonal and people skills with the ability to work and interact with diverse groups of people
Must have valid TX Driver License and auto insurance to be designated as an authorized driver
Proficient in Microsoft Word, Excel, and Outlook
Preferred Qualifications
Master’s degree in Social Work, Counseling, Psychology, or closely related field
LMSW, LCSW, LMFT, or LPC
1 year supervisory or management experience
Bilingual (English and Spanish based on service area and service population)
Physical Demands & Work Conditions
This position requires sitting and looking and using a computer for long periods of time
Position works with children and families and has constant exposure to youth
Lift, push, pull, move up to 50 lbs.
