Kalihi Palama Health Center Care Coordinator Supervisor in Honolulu, Hawaii

Primary Purpose: The person in this position is a member of the Patient-Centered Medical Home where each team member plays an integral role in providing patient-centered health care. The incumbent is responsible for the planning, directing, and implementation of all patient care coordination activities. The Supervisor participates in quality improvement initiatives that enhances patient care and has direct supervision for all 915, 952, and 710 care coordinators. The person in this position is responsible for the direct supervision of the Care Coordination Team and performs care coordination and direct patient care as necessary.Essential Duties: Works in concert with the Director of Clinical Operations in setting and achieving care coordination goals and standards that are consistent with KPHC's overall mission and vision.Prepares and develops care coordination processes, policies, and procedures to improve patient care.Conducts new staff orientation and annual care coordination training as necessary.Periodically audits care coordination programs and services for compliance with established quality standards and provide quarterly report to the Director of Clinical Operations.Communicates and collaborates with the other members of the team to achieve inclusive and seamless patient flow.Assists in evaluating and resolving patient and staffing issues and concerns as they occur.Develops and implements forms for data collection, tracking and documentation of activities and works with other teams to determine effective communication strategies.Maintains active involvement in staffing development and recruitment.Reviews performance, coaches, and sets disciplinary measures for direct reports in consultation with the Director of Clinical Operations and the Human Resources Director.Manages and coordinates patients assigned to her by creating and implementing health care plans, patient education, closing gaps in care, and facilitating other patient needs as necessaryPerforms direct nursing care such as triage, nurse advice, and floor duties and/or covers other departments as necessary.Regular and reliable on-site attendance is required.Other Duties:Perform other related duties as assigned by the Director of Clinical Operations or other members of management.Keeps abreast of development and trends in areas of responsibility.Maintain and improve personal competence.Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.Minimum Qualification Requirements:Skills/Knowledge:Knowledge in leadership and management best practices and ability to effectively lead a team of Care Coordinators.Knowledge and skill in the delivery of community healthcare.Knowledge of community resources, how to access such, and an awareness of government assistance programs.Ability to assess and plan in a multi-cultural context.Professionalism, sound clinical skills and emotional stability.Ability to communicate comfortably with multi-ethnic populations.Flexibility in program development and evolution.Strong organizational skills.Database creation and maintenance also desirableIntermediate ability to use MS Windows including Word, Excel, PowerPoint and Access. Education:Registered nurse, graduate of NLN accredited training program. Bachelor of Science in Nursing required. Advanced Degree in Nursing preferredCurrent RN or license appropriate to area of specialty, if such license is issuedContinuing education current as required by licensing agencyExperience:1 year management experience preferred but not required2 years general experience providing patient care in community or hospital setting.1 year case management/care coordination experience or experience providing health education and outreach activi
