Wellstar Paulding Hospital - We are seeking a IV Room / Sterile Compounding Specialty Pharmacist to join our Team!
Pharmacist Specialty Responsible for the coordination of specialty pharmacy services in assigned area. Areas can include, but are not limited to, the Pharmacy IV Room, OR Pharmacy, Outpatient Infusion Pharmacy. Provides consultative / clinical services including antibiotic dosing of vancomycin and aminoglycosides, initial Coumadin dosing, renal dosing and IV-to-PO conversions. Responsible for providing distributive services. Responsible for adhering to and practicing within the guidelines of Georgia State Pharmacy Law.
Knowledge of Drug Appropriateness:
Evaluating and monitoring drug regimens for appropriateness.
Interpreting physician’s orders and screening them for various types of interactions.
Providing patient and nursing education when requested.
Reviewing medication profiles to maintain accuracy and optimize medication therapy.
Able to perform all duties of a Pharmaceutical Care Pharmacist.
General Pharmacy Practice:
Make workflow adjustments and provide direction to team members working in their area.
Provide leadership for troubleshooting and resolving issues in their area.
Preparing IV Admixtures, compounding, and repackaging a wide variety of pharmaceuticals.
Entering and verifying orders into the pharmacy computer system, and proper dispensing of medications.
Able to follow patients for antibiotic and anticoagulant dosing.
Participates in daily rounds.
Works with other healthcare providers to assist in the patient education for new medications and their side effects.
Assists with medication reconciliation and discharge counseling.
Provides supervision and guidance to all personnel working in their area.
Provides expertise to other team members and practitioners in their area.
Demonstrating skill and competency in patient assessment and providing patient and nursing education when requested.
Complete training requirements with new pharmacists and pharmacy technicians.
Work with pharmacy students as needed.
Required Minimum Education :
Bachelor’s Degree from an accredited School/College of Pharmacy required.
PharmD highly preferred.
Required Minimum Certification :
Licensed and in good standing with the Georgia State Board of Pharmacy.
Required Minimum Experience:
Hospital pharmacy and sterile compounding experience required.
2 years clinical involvement or Board Certified Pharmacotherapy Specialist (BCPS) preferred .
Required Minimum Skills:
Demonstrated pharmacokinetic skills.
Strong order entry and computer skills.
Good oral communication skills.
