These programs are designed to help people continue tolive in their own homes or in other community settings, as an alternative tonursing home care.OLTC provides assessment and ongoing case management to older and disabled lowincome individuals who are in need of long-term care services in their ownhomes or alternative care facilities. OLTC also provides assessments forpeople in need of placement in a skilled facility. OLTC currently providesongoing services to approximately 1300 Larimer County residents outside ofnursing facilities, with the goal of providing long term care in thesettings requested by individuals and their families and utilizing Medicaid inthe most cost-effective manner. ResponsibilitiesSpecifically, this position:Manages a caseload of approximately 100 + individuals on an HCBS waiver andor complete initial assessments for individuals applying for Long Term CareMedicaid programs.Provides crisis intervention as needed to assist individuals in decisionmaking and problem resolution; acts as advocate for the individual.Assists individuals in determining services and supports to meet their needsin the community in an effort to prevent nursing home placement. Assesses forNF placement when appropriate.Coordinates services including scope and duration while ensuring timeliness ofservice delivery.Is able to create a service allocation based upon functional need.Educates individuals regarding Medicaid benefits, programs and non-Medicaidservices in the community.Monitors individuals quarterly or more often to ensure the ongoing provisionof need and satisfaction of services.Works cooperatively with internal Department of Human Service agenciesincluding but not limited to LTC Medicaid Tech's, Adult Protection andOffice on Aging. QualificationsAll candidates will be screened for the following required minimumqualifications. Any candidate who does not meet all of the qualificationslisted as required will be eliminated from the process.Equivalent to a Bachelor's Degree from an accredited college or universitywith major coursework in Sociology, Psychology, Human Services, SocialWork, Public Administration, or a related field required.
