Hunterdon Health Care System Inpatient Family Liaison in Flemington, New Jersey

Position Summary

Inpatient Family Liaison will assist in connecting patients with their families and ensuring patients' needs are met while on the inpatient unit. This role will also work closely with Nursing Staff to help with communication with patients’ families regarding care.

Primary Position Responsibilities

  • Assist in making virtual calls/meetings to connect patients with their families.

  • Assist in answering calls from family and assist with getting care questions answered.

  • Assisting with virtual calls/meetings for discharge information/care plans

  • Schedules virtual and/or in person visits and ensures the visitation guidelines are being followed.

Secondary Position Responsibilities

  • Acting as a companion for patient, ie, helping feed patient meals, getting them water, answering call bells/rounding etc

  • Empting linen cart in room when full

  • Removing full trash can in room when full

  • Restocking items for patient room

  • Tasks charge nurse assigns as needed

  • Runner for supplies and ppe as needed

Reports To: Charge Nurse and Director, Organizational Improvement

Minimum Education Requirements

Required: High School Diploma or Equivalent

Minimum Years of Experience

Required: None

Preferred: Two years in a Customer Service industry; healthcare experience as a Nursing Assistant, Medical Assistant or LPN preferred

License, Registry or Certification

Required: None

Knowledge, Skills and/or Abilities

Required:

  • Working technical knowledge of ipad and making virtual calls

  • Good communication/verbal skills

  • Ability to interact effectively with the public

  • Must wear a N95 mask. Masks will be provided by HMC.
