Responsibilities

  • Deliver pharmaceutical care in the designated internal medicine/cardiology (telemetry) patient care area.

  • Supports work of Pharmacy and Therapeutic Committee (P&T) and assists with planning, development, implementation and maintenance of Formulary at MercyOne.

  • Conducts and/or assists with Medication Use Evaluation (MUE)s and participates in departmental Quality Improvement (QI) efforts.

  • Assists with monitoring and analysis of Adverse Drug Events including medication errors for internal medicine and telemetry patient care areas.

  • Actively participates in and/or leads local and national committees related to medication use.

  • Leads or participates in the development and implementation of multidisciplinary medication-related protocols/guidelines and clinical pharmacy initiatives for internal medicine practice areas.

  • Assists with training and development of Staff Pharmacists as it relates to internal medicine.

  • Provides education to pharmacy, medical, nursing and other healthcare professionals, through lectures, literature reviews and other educational activities.

  • Serve as preceptor for pharmacy students and residents on internal medicine and cardiology rotations and assists with projects for students and residents as needed.

  • Performs other job functions as directed by Clinical Pharmacy Manager

Qualifications

Required education/experience:

  1. Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.

  2. Successful completion of PGY-2 residency in the internal medicine, cardiology, or pharmacotherapy or minimum three years of practice experience in internal medicine.

Required licensure and certification:

  1. Licensed, or eligible for licensure, pharmacist in Iowa.

  2. Board certification within 1 year of hire.

  3. BLS and ACLS certification within 6 months of hire.

Required Minimum Knowledge, Skills and Abilities

Knowledge:comprehensive knowledge of pharmacology, therapeutics, kinetics, therapeutic drug monitoring and critical literature evaluation as it relates to patient care within the area of specialty practice.

Skills:

  1. Effective communication with patients and other healthcare professionals.

  2. A high level of analytic ability to research and evaluate drug information, interpret patients specific information, develop education material, conduct medication use evaluation, and resolve medication related problems.

  3. Commitment to teaching, mentoring and coaching students, residents and pharmacists

  4. Proficient in use of computers, software for pharmacy and healthcare applications, specific pharmacy practice technology and equipment
