CommonSpirit Health House of Mercy Project Coordinator in DES MOINES, Iowa

Overview

MercyOne Central Iowa operates four not-for-profit, Catholic medical centers in Des Moines, West Des Moines, Newton and Centerville and two specialty hospitals – MercyOne Children’s Hospital in Des Moines and MercyOne Rehabilitation Hospital in Clive (965 beds total) – along with more than 20 additional facilities that house more than 50 primary care, pediatric, internal medicine and specialty clinics. Responsibilities

  • Establishes rapport and develops a therapeutic relationship with patients. Provide brief interventions to patients and/or referrals to appropriate substance use disorder treatments for patients. Therapeutic efforts are primarily directed towards addressing issues related to substance use disorders. Maintains appropriate professional boundaries with all persons involved.

  • Increase collaboration within the MercyOne health system, in coordination with community partners, to identify and assist patients impacted by opioid use disorder.

  • Conduct universal SBIRT prescreening of patients (ages 12 and up) in the pain clinics, inpatient pain consults and trauma department.

  • Develop and implement a chronic care model in caring for patients identified with an opioid use disorder in this SBIRT program to ensure patients have a supportive and evidence-based interactions with prepared, proactive, collaborative practice team.· Conducts recovery related educational sessions and groups providing information and instruction.

  • · Provides crisis intervention and conflict resolution assistance.

  • Participates in the assessment/screening utilizing a compassionate and empathetic manner to ensure all patients entering the program feel welcome, appreciated and understood. Helps ensure all patients feel a focus on collaboration and engagement from the very beginning of their involvement with House of Mercy.

  • · Develop a plan and implement two community outreach and awareness events, primary in low-income neighborhood and rural communities.

  • Develop and conduct a needs assessment and interest survey regarding SBIRT expansion to key affiliate hospitals, clinics and dentists.

  • Conduct staff training on the new work processes. Review effectiveness of processes on an ongoing basis, and make changes as needs are identified.

  • Participates in regularly scheduled trainings and care meetings to discuss/update staff on patients progress, status, and treatment needs. Professionally communicates with other program staff to promote team collaboration, input, and involvement in problem solving/resolution insuring consistency in the implementation of treatment plan goals/objectives.

  • Effectively communicates patients service needs with community agencies, schools, courts, service providers of significant others, and mental health/other health care professionals to provide an extended, high quality continuum of care.

  • Responds to volatile situations by mediating to resolve issues and to model appropriate crisis intervention and problem-solving techniques.

  • Ensure SBIRT screening and subsequent interventions and screenings are culturally and linguistically appropriate: referring patients to a bi-lingual counselor when language needs are identified; provide resources materials in language of patients, utilizing MercyOne’s onsite translation and interpretations services; and ensuring counseling staff are trained to gather information about cultural factors that are important to patient’s recovery.

  • Cultural awareness, knowledge and skills are implemented throughout House of Mercy and guide clinical decision-making when treatment is provided.

  • Develop an effective patient referral process to ensure a warm hand-off between health care provider and the substance use treatment.

  • Track number of patients screened, and those who receive brief interventions and referred to treatment.

  • Disseminate Naloxone to patients when high risk for overdose is identified

  • Refer patients to substance use disorder treatment provider for appropriate services, including MAT.

  • Provide support in accessing treatment for referred patients who are experiencing barriers to accessing care.

Qualifications

High School Diploma or GED required.

  • Bachelor’s degree in a human services related discipline from an accredited four-year program, required.

  • Licensure in substance use disorder counseling - CADC Certification

  • Project coordination experience and experience in SBIRT and opioid focused treatment.

  • Required to drive to Polk/Warren Counties

  • Valid Driver’s/Chauffeurs License required, must meet Mercy’s Motor Vehicle Safety Standards, must be at least 18 years of age and be eligible to drive per Iowa state law.

  • Proof of completion of Mandatory Reporter abuse training specific to population served within thirty (30) days of hire.

  • Ability to understand medical terminology and communicate effectively with healthcare professionals.
