Home dialysis (in particular, peritoneal dialysis or "PD") offersunique and significant clinical and lifestyle benefits to ESRD patients.DaVita-the largest provider of Home Dialysis and leader in Home Dialysisinnovation-is on the path to double our Home Dialysis business by achieving 1in 4 patients on Home by 2025.As part of our commitment to 1 in 4 patients on Home-and in recognition of thefact that additional clinical support is essential to ensuring "qualitygrowth"-DaVita is significantly expanding the level of support to ourphysician partners. As part of this support expansion, DaVita is activelyhiring additional National Physician Home Dialysis Experts to engage directlywith prescribing nephrologists and Home program Medical Directors on suchcritical issues as patient selection, Home prescription management,advancing clinical quality metrics, program safety, and maintainingpatients on Home therapy, as appropriate.The National Physician Home Dialysis Experts will report directly to the ChiefMedical Officer for Home Dialysis, and will work closely with the NationalPhysician Home Support Director, who will coordinate their activities withfield leaders, target physicians, and Home program Medical Directors.These experts will also work closely with our National Home Modalities Team,composed of highly motivated individuals with previous leadership experiencefrom DaVita field operations, top-tier consulting firms, investmentbanks, and other healthcare organizations who lead our Home Dialysis business.In working with key physician practices, National Physician Home DialysisExperts will collaborate with local field leaders (Division VicePresidents, Regional Operations Directors, Home Group Leads, and HomeDivision Leads) and clinical leaders to understand practice needs,barriers to Home dialysis utilization, and gaps in Home dialysis physicianknowledge. The National Physician Home Dialysis Expert will assist inconducting a diagnostic, then work with practice physicians to set mutualgoals and develop a plan for achievement. It is anticipated that the NationalPhysician Home Dialysis Expert will continue in-depth engagement with thesetarget practices for a period of at least 3-4 months, tracking progress andproviding clinical support, as needed. Much of the support will be deliveredvia phone, although it is likely that monthly in-person support will also berequired in certain settings.In addition to supporting prescribing physicians, National Physician HomeDialysis Experts will also work with national and local clinical leaders tosupport improvement in clinical outcomes at select DaVita clinics. In thiscapacity, National Physician Home Dialysis Experts will use clinicalperformance tools to assist Home clinic Medical Directors in assessing areasof clinic opportunity and developing clinic performance improvement plans.National Physician Home Dialysis Experts may also conduct various speakingengagements within DaVita and among the clinical community, as well as holdmeetings with various stakeholders to help accomplish transfer of knowledgeand best practices. National Physician Home Dialysis Experts may be involvedin other research, academic, and clinical pursuits which impact theoverall quality care delivered by DaVita Home programs and representativephysicians.Success will be measured by (but not limited to) demonstrating improvementin Home penetration, Home new starts rate, and Home patient loss rate fortarget physician practices and improvement in facility quality metrics andpatient safety at DaVita clinics supported.Essential Qualifications:Nephrologist in current practice with at least 5 years of experienceDemonstrated experience in delivering lectures and / or teaching physiciansin Home dialysisHome dialysis practice penetration of at least 15-20% and affiliated Homeprogram quality metrics >= mean outcomes targetsHighly skilled in PD prescription management and helping to address patientbarriers to time on therapy (HHD skill a plu
