Under the direction of the Director of Nursing or designee , the Home Infusion Nurse provides direct patient care and education to patients and families regarding infusion therapy administration while always maintaining the highest level of integrity. Completes patient assessments and coordinates patient services with other internal departments. They also perform other duties as assigned.

Primary Responsibilities:

  • Performs direct and indirect care to patients in the home or alternate setting

  • Develops and ensures appropriate distribution of consumer/patient educational materials

  • Assess patients during initial visit for current medical and psychosocial needs. Develop plan of treatment and plan of care

  • Completes care conferences as assigned by DON

  • Delivery of medications and supplies as requested

  • Discharges patients per policy

  • Coordinates lab draws and delivery

  • Performs well independently and solve problems

  • Communication to pharmacy regarding patient supplies/medication inventory

  • Provides telephonic and face-to-face consumer educational services

  • Confers with customers (physicians, patient/family, health care personnel, etc) during the planning process to establish mutual goals

  • Document in computer software system

  • Maintain adequate nursing supplies in nursing bag and to be available at every nursing visit

  • Provide on-call coverage and participate in the on-call rotation

  • Communicates the Patient Bill of Rights and Responsibilities to the patients and caregivers, obtains signatures on the Agreement for Services, consent, and other forms as required by law

  • Participates as necessary in care conferences and meetings related to DSIG patients

  • Program and tool development for DSIG programs

  • Timely and appropriate documentation as indicated

  • Local and regional representation with non-profit organizations

  • Keeps current regarding community health issues and regulations

  • Development of educational programs and symposiums locally and regionally

  • Completes required reporting and accounting of time and expenses within the required time frame

  • Demonstrates a high degree of personal responsibility regarding accuracy and quality of work

  • Compliant with all State, Federal and Local regulations regarding nursing practice and the provision of patient care

Required Qualifications:

  • Nursing Diploma, Associates Degree Nursing (ADN) or Bachelor of Science in Nursing (BSN)

  • CPR Certification

  • Valid license, in good standing, to practice as a Registered Nurse in the state employed

  • Must have access to reliable transportation

  • 1+ years of nursing experience

  • Health screen requirements:

  • TB Screen Annually

  • Physical upon hire

  • MMR Titers

Preferred Qualifications:

  • Home infusion experience

  • Experience administering infusion therapy in a home or alternate-site setting

  • Knowledge of and experience with Microsoft Office and electronic medical records (EMR) programs

  • Knowledge of the Infusion Nursing Society (INS) standards of practice, policies and procedures

  • Knowledgeable of HIPAA patient privacy and data security requirements, OSHA employee protection regulations, and accreditation standards for home health and home infusion pharmacy (or other relevant accrediting organizations)

  • Proficient with Microsoft Office

  • Flexible to work a varying schedule including two weekends per month

  • Evening availability until 10:00 PM ET

  • Proficient with electronic medical record documentation

  • Willing and is able to work with an interdisciplinary team of professionals including pharmacists and technicians, physicians and other licensed prescribers, registered dietitians, home health agencies (HHAs), sales representatives, and others as needed to provide comprehensive care and support of patients and caregivers

  • Willing and able to work independently in home or alternate-site settings

  • Willing and able to assume a flexible work schedule

  • Willing and able to meet attendance, overtime, after-hours on-call, and other reliability requirements of the position
