The Registered Nurse provides nursing services in the Health Center in accordance with agency standards and policies. Responsible for ensuring that confidentiality is strictly maintained concerning all agency related information in accordance with HIPAA regulations. Collaborates with other healthcare professionals, community agencies, patients family members, and others involved in a patients care as appropriate. Supports the Patient Centered Medical Home model of care by working with other elements within the practice to provide team based care. ESSENTIAL FUNCTIONS/RESPONSIBILITIES:• Responsible for collecting and recording medical data including anthropometric measures and vital signs.• Obtains nursing history and records findings in the medical record.• Carries out and records procedures, treatment, immunizations and administration of medication as ordered by physicians, nurse practitioners and physician assistants.• Provides health education for patients and their care providers and documents appropriately in medical record.• Attends scheduled meetings.• Serves as a resource to staff on health related matters.• Assists in maintaining safety standards and infection control policies in the clinic.• Provides phone advice for the management of health problems.• Triages medical problems to determine the need for medical attention.• Functions as a member of an interdisciplinary team.• Maintains professional competence through continuing education, inservice programs and appropriate learning experiences.• Performs CPR.• On call responsibilities as assigned.• Complies with all agency policies and procedures.• Other duties as assigned.

• Associate Degree in Nursing from an accredited program required with a minimum of two (2) years of experience in a clinical setting; BS Degree in Nursing from an accredited college or university preferred with one (1) year experience in a clinical setting.• Experience working with a special needs population helpful but not necessary. • Current NYS Nursing License and Registration required.• CPR Certification.
