Do you have what it takes?Additional InformationDepartment: Level Three-AmbulatoryHours per week: 40, eligible for benefitsShift: M-F, 8a-5p with off hour coverage, including nights, weekendand holiday coverage depending on the needs of the patients and organization.Job OverviewThe Clinical Program Coordinator is an expert nurse who assumes responsibilityand accountability for program coordination and care coordination for aspecific patient population. The Clinical Program Coordinator works acrossdisciplines to develop, support, and implement programs and strategies forcare to improve health outcomes for a specific program or population ofpatients. Areas of focus include program development, communication,sharing of information, hand offs, transitions, advocacy, patient andfamily guidance, education, resources, and population specificexpertise. The Clinical Program Coordinator provides guidance and support tostaff in the management, provision and coordination of care in their area ofexpertise. The care coordinator assesses, plans, implements, coordinatesand evaluates the plan of care in partnership with the family and othermembers of the health care team.
