The Case Managercollaborates with the patient, family, other healthcare team members,community providers, and community resources and payers, resulting in anappropriate level of care, placement, length of stay, follow up care,outcomes and reimbursement.ResponsibilitiesPOPULATION SPECIFIC CARENo Direct Patient CareESSENTIAL FUNCTIONSAn employee in this position may be called upon to do any or all of thefollowing essential functions.
